FARMINGDALE, N.Y. | The Farmingdale State College Sports Medicine department will be holding a physical date on Wednesday, May 17, 2023, from 3:00pm-4:30pm located in the Nold Hall Athletics Complex. No Appointment is Necessary. Please bring your cell phone and come to the main lobby when you arrive. All Athletes participating in the 2023-2024 athletics seasons are welcome, this includes any returners, new student-athletes and transfer athletes.
All Student-Athletes will have their physicals completed, as well as an Orthopedic screening and Concussion baseline testing performed.
** Any athlete that is under the age of 18, please be accompanied by a parent or legal guardian, or have a letter stating that you are allowed to participate in the physical, ortho screening and concussion baseline testing signed by your parent or legal guardian **
If you are a new or transfer student-athlete, please visit NEW STUDENT-ATHLETE PAPERWORK to obtain your sickle cell verification and cardiac questionnaire forms. The sickle cell form will need to be completed by your own physician and lab results attached, prior to being completely cleared. Transfer athletes may request copies from their prior school, if testing was completed last year. Completed sickle cell forms can be emailed to FSCsportsmedicine@farmingdale.edu or handed into the Athletic Training staff located in room 106 of Nold Athletics complex, Monday-Friday from the hours of 10:00 am -3:00pm.
Sportsware will be reset as of May 22, 2023. All new/transfer and returning student athletes will need to complete all necessary information and upload a new copy of their insurance cards. All Farmingdale State College Athletes will have to have a concussion baseline test done. This needs to be completed prior to any workouts or practices. Baseline testing will be completed by the Athletic Training Staff during the available hours of 10 a.m. – 3 p.m. Monday through Friday in Nold Hall room 106.
****If any student athlete cannot make the May 17th physical date, physicals can be done by your own physician or at the FSC Health and Wellness Center (by appointment only). Call 934-420-2009 to schedule an appointment. Physicals must be completed and dated after May 17, 2023. If insurance cannot cover your physical in this time frame please schedule an appointment with the FSC Health and Wellness Center.*****
