PHYSICAL EXAMINATIONS
If you are having a complete entrance physical examination, you must bring copies of your immunization records with you.

**This year, all paperwork including physicals, immunizations (new/transfer athletes only), cardiac questionnaire(new/transfer athletes only), and the meningococcal vaccination response form (new/transfer athletes only), will be collected by the Athletic Training staff. Athletes will no longer be required to submit this to Health in Wellness in order to be cleared.**
INSURANCE COVERAGE
ALL athletes are required to have medical insurance. If they do not, they can apply through the New York State Health Plan Marketplace.
*Farmingdale State College will assume no responsibility whatsoever for the payment of, or authorization to pay, medical expenses resulting in injuries that occur while participating in intercollegiate athletics at Farmingdale State College.*
