PHYSICAL EXAMINATIONS
Please call for appointment during our scheduled clinics.
If you are having a complete entrance physical examination, you must bring copies of your immunization records with you.

Note: If any returners are not added to the current team roster, they will be charged a fee for their physical. Only if your name is on the list at the center will you be allowed to get a free physical.
FSC Sports Medicine will be holding an Ortho screening and Physical date on Saturday July 30th. We would like to have everyone here on that date, if possible. We are giving you the option to get your physical done on your own prior to this date, so you can just come in for the ortho screening and to complete your concussion baseline test. Health and Wellness will be open all summer doing physicals on Wednesday, Thursday and Friday, by appointments. All Athletes can call to make an appointment, insurance is not needed for these physicals. We are looking to eliminate the half-year clearances, so if you cannot get a physical with your primary doctor between 6/1 and 8/1 please utilize Health and Wellness. Appointments can be made at 934-420-2009.
Please note: New and Transfer Athletes will need copies of their Immunizations (MMR/Measles/Mumps/Rubella/Hep B/Meningococcal (resident students only)/TDAP (within 10 years) as well as verification of their sickle cell trait status. These forms can be found in the New/Transfer Student Athlete paperwork
IF any Athlete cannot make the July 30th date, please email us at FSCSportsmedicine@farmingdale.edu to discuss options.
**This year, all paperwork including physicals, immunizations (new/transfer athletes only), cardiac questionnaire(new/transfer athletes only), and the meningococcal vaccination response form (new/transfer athletes only), will be collected by the Athletic Training staff. Athletes will no longer be required to submit this to Health in Wellness in order to be cleared.**
INSURANCE COVERAGE
ALL athletes are required to have medical insurance. If they do not, they can apply through the New York State Health Plan Marketplace.
